The form is embedded directly on the site and is hosted by JotForm, which uses strict security protocols to keep information safe. Questions are designed to gather the information needed to understand counseling needs and support requirements. Step 3: Submit and Review Reports Once the assessment is submitted, a report is generated immediately.
