Thank you for taking the time to complete the MCARS. Your responses are important and will help provide clear guidance and next steps. The assessment is straightforward, takes only a few minutes, and all information is handled securely through JotForm.

The MCARS is designed to be used as part of the decision making process when school teams are determining the need for mandated counseling services and interventions as part of a student’s educational program. Mandated counseling services are generally part of the student’s IEP or 504 Plan. MCARS should be completed by gathering input from various members of the school team (e.g., teachers, school psychologists, school counselors, other related services providers, nurses, administrators, etc.) to ensure comprehensive data-based decision making. Mandated counseling is generally offered when the student's functioning in school is significantly impacted by social, emotional, or behavioral concerns. The MCARS assesses students' functioning in several areas.

• Most assessments take about 10 minutes to complete.
• Questions are simple and research-based.
• Make sure to notate who is taking the assessment, since names are not entered into the system.

• Results are generated immediately when the MCARS assessment is complete.
• The score that you receive indicates a numerical sum of your ratings on all scale items. 
• The report includes recommendations and next steps to guide student support.
• A copy of the report will be sent to your email.
• You can print your email confirmation and add the student’s name manually for your records.

The assessment is hosted on JotForm, which uses advanced security protocols to safeguard information. MCARS also follows standard practices to ensure your data is only accessible to authorized users.
